By submitting this transfer request, I agree to and certify the following:

I am the legal owner of the building and am requesting the transfer of ownership for this roof Guarantee in full, including all areas and sections of roof currently listed. Please note that this Guarantee may not be eligible for a partial transfer.
The New Owner must be listed as it is recorded on the deed (i.e. Company XYZ, LLC) and cannot be “in care of” or a management company.
An inspection of the roof by a JM Field Technical Representative may be required. A non-refundable transfer fee of $1000 will be sent to the “transfer contact” listed above. If upon review of this application, JM waives the required inspection, JM will reduce this fee to $500. If JM does not receive payment of this fee within 90 days, we will consider your request withdrawn.
Any roof deficiencies that are identified during the JM inspection not covered by the Guarantee will be reported to the Owner Contact and should be addressed as soon as possible.
Transfers are subject to JM's sole and ultimate discretion and that accepting the application and non-refundable transfer fee is not an indication of JM's acceptance or approval of a transfer application
Once the transfer process is complete, a copy of the Guarantee listed under the new owner's name will be issued.
